fd2e5e226d Removed the $.append() calls that were conflicting with document.write() 
on the demo server.  Instead, generating the div on the fly and not
appending it to the DOM.

9a4b524db7 Hacked the PMA_generate_slider_effect() code to work with jQuery. Ajax 
queries were failing due to the use of document.write(). Need to find a
better solution with $.append()
